What is the procedure for getting the investigations?

What is the procedure for getting the investigations done by an
empanelled diagnostic centre?

No Permission is required for getting listed Investigations done at empanelled Diagnostic Centres, if prescribed by CGHS Medical Officer/ CMO Incharge or Government Hospital specialist.

The following documents are required to be submitted:

a. Self attested copy of prescription of CGHS Doctor /
Government specialist

b. Copy of CGHS Card of the patient and main card holder. Original Prescription and Original Cards are to be produced at the centre for verification. Investigation can be done within 14 days of advice only.
